Confidentiality.

Your treating clinician is a qualified and registered professional. As part of providing a service to you, the clinician will need to collect and record personal information from you that is relevant to your assessment and/or treatment. This information remains the property of FLOW.

All personal information gathered by the clinician in the provision of services will remain confidential and secure except when it is subpoenaed by a court; it is necessary to provide details to enforce the legal rights of FLOW; when failure to disclose information would place your or another person’s life in imminent risk; or, when your prior approval has been obtained.

All clinicians at FLOW are mandated to report suspected child abuse and neglect. South Australian psychologists are also mandated to report nurses at risk and possible firearms risks.  It is a requirement of Medicare, that a summary of your treatment is provided to your referring doctor if you are being treated under Mental Health Care Plan.

If you would like a copy of the information held in your personal file, a request in writing must be submitted to the Practice Manager. Information will be provided in the form of a treatment summary. Raw data and case notes will not be released. If you attend as a couple or family, all information is jointly owned and all consenting parties must approve any requests for access. Administrative fees will apply to the preparation and provision of information.

If you child is accessing an “on-site service” (eg. at their school) the school may be made aware that they are attending an appointment with FLOW (no other information will be provided unless consent is given), in order for the school to maintain proper duty of care.
